To download forms you will need Adobe Acrobat reader© to view and print these
forms if you do not have this program you can download it by
following the below link. After downloading and installing you
will be able to view and print any form posted here. And a reminder don't
forget to sign your forms.
A note about printing these forms, when you choose to
print look at your options when the print dialog appears and look
for a option that says "shrink oversized pages to paper size" that
way the whole page prints, even though the whole page appears on
the screen on some printers the whole page does not print, this
should cure it for all printers if the option is available under
the print screen that appears. To return to this page just hit
the back button on your browser.

This is a 2 page basic form, you can
fill out when you open it with Acrobat Reader on your computer,
use your tab key to move around the entries on the form , print
it and you can either fax or mail it to the Village office or
bring the completed form during regular business hours or use

for Building/Zoning Permit
(Recommended) This is a 8 page Building/Zoning Permit
application, it has the 2 page basic form to fill out for the
application, various diagrams of what should be noted on for
showing the design or structure for what the permit is needed
for, or just print then fill in the blanks on the pre-designed

3 Page Application For a Variance
or Appeal
This is a 3 page Application to the Zoning Board
of Appeals for a Variance or Appeal, along with the short
Environmental assessment form, fill out if needed using your
acrobat reader or you can fill out by hand after you have
printed the forms. Then bring or fax, or mail the completed
